Intake form builder

HIPAA-compliant patient intake forms, and the workflow around them

Most HIPAA form builders stop at the form: answers land in an inbox or a PDF someone retypes. Panaceum builds the form and what happens next (check-in queues, staff review, follow-up tasks) as one app, with answers stored in your own encrypted database.

Early access. Panaceum is invite-only while we open up. You can build and preview HIPAA apps with synthetic data now; publishing with real patient data on HIPAA hosting under the BAA opens when plans launch, with no rebuild. Join the waitlist to request access.

Built from one description

  • Any form you describe

Demographics, history, medications, consents, screening questionnaires such as PHQ-9 or GAD-7, and custom specialty forms.

  • Check-in queue

The front desk sees who has completed intake, who is waiting and who needs help, in real time.

  • Answers where you work

Answers become records in the app, visible to the roles that need them, instead of emails and attachments.

  • Validation and logic

Required fields, conditional questions and range checks, described in plain words.

  • Audit trail

Who opened or changed each submission, and when, logged automatically.

  • Accessible by default

Every page passes automated accessibility checks, so patients can fill in forms with a screen reader or on a phone.

Example descriptions

Paste one into the prompt box on the home page, or write your own in the same style: who uses it, and what they need to do.

HIPAA safeguards included

Intake answers are PHI, so the form, where answers are stored and anyone who can see them fall under HIPAA. Panaceum keeps them inside your HIPAA app: encrypted at rest, visible only to the roles you choose, audit-logged, and hosted under a BAA. If you only need a single standalone form, a HIPAA form vendor may be enough; if you need a workflow, build the app. See the HIPAA website builder for putting forms on your practice site.

  • Access rules per role

Every page, record and field is scoped to the roles in your plan: patients see their own records, staff see their panel. Changes show up as a security diff before release.

  • Audit logging from day one

Every read and write of health data is logged with who, what and when. Audit logs are stored where they can't be edited or deleted.

  • Encryption everywhere

TLS in transit and encryption at rest for databases, files and backups, with optional field-level encryption for identifiers such as SSNs and member IDs.
